2-1-1 San Diego has grown from a small program of the United Way, selling a printed hard copy resource guide, to being the region’s trusted source of information. With the ability to connect people to more than 6,000 community resources 24 hours a day 365 days a year, we are able to help more than a quarter of million individuals each year.

2-1-1’s mission is to serve as a nexus to bring the community (organizations) together to help people efficiently get appropriate services, and provide vital data and trend information for proactive community planning.

Using Information and Referral services as the foundation, we’ve built a multi-tiered approach to service delivery offering deeper levels of care including direct assistance programs, personal navigations services and ongoing care coordination.

2-1-1 successfully harnesses the power of technology and innovation to transform care. The 2-1-1 NextGen solution builds upon that success to advance connections and create a communitywide ecosystem of unified organizations exchanging data, outcomes, resources and solutions.

Enrollment Center

2-1-1 pioneered telephonic signatures for public benefit programs
applications in the State of California and has submitted more than
30,000 applications for CalFresh, Medi-Cal and Covered California,
bringing $38,000,000 in economic stimulus to San Diego.

Health Navigation

2-1-1 identifies, connects, empowers, educates, and advocates
for individuals and families with complex health needs. This
top tier program provides care coordination addressing health
and social aspects of the whole person.

Military and Veteran Services

2-1-1 answers the Courage to Call peer-to-peer helpline offering
navigation and care coordination serving as the single point of
access for veterans, active military, reservists, national guards
and their families. 2-1-1’s Military and Veteran program is the nexus
for resources, catalyzing collaboration and maximizing impact.

2-1-1 San Diego is the voice of the people.

2-1-1’s real-time client data provides valuable demographic and trend information to assist
key leaders, foundations, and policy makers in community planning.

In times of disaster 2-1-1 San Diego is there.

By partnering with San Diego County’s Office of Emergency Services, CAL FIRE and the region’s public safety agencies, 2-1-1 provides the public with a place to turn for non-emergency needs during times of disaster. 2-1-1 tracks and reports community unmet needs in real time as the central communication point and feedback loop during disasters and recovery efforts.
